Remember before you take out the policy:

• If you are a small employer or self-employed person

  • ASISA Salud PYMES is aimed at small and medium-sized companies and self-employed people.
  • The policyholder/employer (the payer of the premiums) must submit the CIF number (tax identification number) in the case of a company, or evidence of registration under the Special Regime for Self-Employed Workers (RETA) in the case of a self-employed person.
  • Minimum of 2 insured persons on the policy.

• Proving your status as employee or close relative

  Employees and close relatives (spouses and children) of the policyholder/employer may be insured persons.

  • You may prove your status as employee through the following documents:
    • Social Security TC2 form.
    • Employee payslip.
  • You may prove your status as close relative through the following documents:
    • Family book
    • Common-law marriage registration certificate
    • Voter registration certificate
    • Cohabitation certificate
    • Banking contract
