BY ORDINARY MAIL OR IN THE NEAREST OFFICE

Download here the change of entity request form.

Remember to mark ASISA in the field "ENTITY"

Send or submit the form form in the nearest office of ISFAS, together with your original affiliation number

CHANGE YOUR INSURANCE COMPANY ONLINE

In order to make the change online you need an electronic DNI and the personal password as well as the required software installed on your computer.

Once you have the software installed you have to go to the website of ISFAS to start the process.

Choose the option "Cambio ordinario de entidad sanitaria" and follow the steps to fulfill and send the ordinary change form.
