CHANGE TO ASISA AND ENJOY THE BEST HEALTH CARE
Below we provide information about the options and the steps you have to follow to complete the change:

BY ORDINARY MAIL OR IN THE NEAREST OFFICE

Download here the change of entity request form.

Fill and print the form with your information. Do not forget to mark with an 'X' the box ASISA in the field "entidad a la que desea ser adscrito"

Once printed you must go to the nearest office of MUGEJU or send it by mail to the office MUGEJU

In the offices you will be able to take the new affiliation document of MUGEJU. With that document you and your beneficiaries will be accredited as insured in ASISA.
